Writer's Submission Checklist for Individuals

Please Make Sure You Have Completed the Following:

  1. ____ You may only submit one (1) pilot script. Make two (2) copies of your pilot script for submission.

  2. ____ Send two (2) separate original copies of the Submission of Material - Release and Agreement.

    Note: To view and print the agreement, you will need the free Adobe Acrobat Reader, which can be downloaded from the following link: http://www.adobe.com/products/acrobat/readstep2.html

  3. ____ Each copy of the Submission of Material - Release and Agreement must be signed and notarized.

  4. ____ You must completely fill in the blanks in Paragraph 1 of the Submission of Material - Release and Agreement.

  5. ____ You must include your address, phone number and email address on the signature page of the Submission of Material - Release and Agreement, so that we may contact you.

  6. ____ Include a cover letter indicating your desire to participate in the FOX Writers Initiative.

  7. ____ Have your script read and evaluated by an individual with considerable industry experience (see FAQs section for details).

  8. ____ Have the evaluator write a letter of recommendation on the letterhead of the company (or school) with which (s)/he is affiliated. Contact information must appear on the letter. The letter must also state the following:

    1. the name, title, telephone number and email address of the evaluator;

    2. his or her qualifications as a reader and evaluator;

    3. that he or she has read the script which was submitted and determined it to have the elements of a well-developed, professional television pilot script; and

    4. the evaluator must sign evaluation letter.

  9. ____ Attach two (2) copies of your resume and/or bio with your name, address, telephone numbers and email address. An email address is necessary for further communication from FOX.

  10. ____ Keep a copy of all material (your pilot script and the Submission of Material - Release and Agreement) you are sending. Submitted materials will not be returned.

  11. ____ Send your submission together with two (2) original signed and notarized copies of the Submission of Material - Release and Agreement to:
